Accident & Incident Reporting

Coaches must report each accident within 24 hours when a FREESTYLE CANADA member is injured and requires medical attention during a sanctioned activity.

In the event of a head injury and/or any symptoms of a concussion, FREESTYLE CANADA Member Clubs are responsible for following the FREESTYLE CANADA’s Concussion policies and procedures.

Below is the form to be filled out in the case of an incident involving two or more parties and/or in the case of property damage. This is also used in the case of EMERGENCY related situations.

FOR THOSE THAT HAVE PURCHASED SAIP INSURANCE:

IMPORTANT ACCIDENT INSURANCE INFORMATION

  1. is only valid during Freestyle Canada sanctioned activities and competitions, including travel to and from the location of a Freestyle Canada sanctioned activity, and coverage 24 hours per day during the time period.
  2. SAIP will cover any gaps in coverage or remaining medical expenses in addition to providing worldwide assistance in medical case management. SAIP provides tertiary coverage. In the event of an accident, claimants must first claim medical incurred expenses through their Canadian Provincial healthcare plan and private health care plan (e.g., employment medical benefits). ​
  3. SAIP Members are solely responsible for initiating an in-country or out-of-country claim with Zurich. Freestyle Canada will not manage claims on behalf of the claimant (see Insurance Claim Process for Membership).
  4. Members are able to upgrade SAIP online through the registration system prior to training, competition or international travel.
  5. The club/coach must submit an Out of Country Sanction Request Form” and be approved by Freestyle Canada to receive coverage.

Coverage is coordinated with any other policy and benefits are payable only for excess charges over and above any amounts payable or collected from the government healthcare and any own/parent/spouse employment insurance plan and/or private individual insurance plan.

TYPES OF SAIP
  1. Class Ia: Mandatory for FIS Licensed National/Development/Provincial Team Athletes who travel out of country for more than 30 consecutive days up to 120 consecutive days. Recommended for coaches and discipline officials who travel out of the country for more than 30 consecutive days up to a maximum of 120 consecutive days. In order to be eligible, an active Government Health Insurance Plan (GHIP) must be kept for the duration of the trip. Coverage is for consecutive days out of country and returning to Canada will reset the consecutive days traveled.
  2. Class II: Mandatory for all other FIS Licensed Athletes and Coaches, Non-FIS Athletes and Coaches  and recommended for Discipline Officials who travel Out of Country up to 30 days.
  3. Class III: Highly recommended for Canadian Athletes/Coaches and Officials who travel outside of   home Province but who do not travel out of Canada.
  4. Class IV: Mandatory for non-FIS Athletes, Coaches and Officials who travel to the U.S. only for training and competition, for a maximum trip duration of 7 days. NOT VALID FOR TRAVEL OUTSIDE OF THE USA.
  5. Class V: Mandatory for non-FIS Athletes/Coaches and Officials who travel to the U.S. only for training and competition, for a maximum trip duration of 14 days. NOT VALID FOR TRAVEL OUTSIDE OF THE USA

Injuries Within Canada

STEP 1.

In the event of an accident or injury requiring treatment, the member or supervising coach of a member who purchased SAIP (CLASS 3 (in-country), 1, 2, 4 or 5 (out-of-country)) Accident Insurance*, must contact Zurich IMMEDIATELY following the accident, if the accident requires any sort of medical treatment.

Zurich Canada Travel Insurance c/o Zurich Travel Assist will facilitate and/or recommend a course of action for treatment, and will subsequently aid in claim support. Please ensure that you receive a claim number from Global Excel and make note of it, for future reference. It is Zurich Canada Travel Insurance c/o Zurich Travel Assist that will provide direction as to the procedure to complete a claim for medical reimbursement if needed.

They are the primary contact in the case of injury, where the member has purchased SAIP Accident Insurance from FC.

STEP 2.

An Accident Report Form must be submitted as soon as possible after the accident, but no later than 30 days after the accident.

STEP 3.

If the member did not purchase any level of SAIP Accident Insurance, then the accident report form is filed and no further action will be taken.

*Please note that the FC’s accident insurance is “excess insurance”, which means that you must exhaust all other valid insurance coverage, from your Provincial/Territorial healthcare system first, followed by any personal accident insurance (Blue Cross, benefits through parents or work, etc.) before attempting to make a claim through the AIG/FC.

Injuries Outside of Canada

STEP 1.

All athletes and coaches MUST purchase the Out-of-Country SAIP (CLASS 1, 2, 4, or 5) Accident Insurance* (depending on the length and frequency of travel) BEFORE they take part in any freestyle activities outside of the country. These freestyle activities must also be SANCTIONED by FC, which is the responsibility of the club and/or coach.

STEP 3.

Zurich Canada Travel Insurance c/o Zurich Travel will coordinate with the injured member or supervising coach, as well as with the treatment facility to either facilitate payment directly to the care facility, or inform the member/coach on how to make get reimbursed.

STEP 4.

The FC accident Report form and the Incident Report form must be submitted as soon as possible after the accident, but no later than 24 hours after the accident.

*Please note that the FC’s accident insurance is “excess insurance”, which means that you must exhaust all other valid insurance coverage, from your Provincial/Territorial healthcare system first, followed by any personal accident insurance (Blue Cross, benefits through parents or work) before attempting to make a claim through the FC.

Chronic Injuries

A chronic injury is either a recurring injury or an injury that either requires treatment over a long period of time. In either case, an accident form from the initial injury must have been submitted to FC, no matter the severity of the injury, at the time of the accident.

STEP 1.

In the event of an accident or injury requiring treatment, the injured member must follow the process outlined above in either the “Injuries WITHIN Canada” or “Injuries OUTSIDE OF Canada”, in order to document and report any incidence of injury, that may later develop into a chronic injury.

STEP 2.

The member’s physician (or the treating physician) must write a note recommending further treatment (e.g. physio., massage, etc.), if the injury is thought to require further treatment.

STEP 3.

Member pays for all subsequent treatment and keeps all the receipts.

STEP 4.

The member must first claim any subsequent treatment through their Provincial/Territorial health care, followed by any personal insurance options (if available). Once those options are exhausted, and the member requires more treatment, the member will contact FC to start a claim, but only if that member purchased the correct level of SAIP Accident Insurance from FC before the accident occurred (see above for Injuries Within and Outside of Canada).

STEP 5.

FC will coordinate with Zurich Canada Travel Insurance c/o Zurich Travel and facilitate the claim process. Any amount not covered by Provincial/Territorial Health care or personal insurance will be covered by FC’s insurance plan, up to the amount in the policy.*
